Root Cause Analysis — Ammonia Leak Compressor C-101
On 15 March 2026, an ammonia leak was detected at the flange of compressor C-101 in the Ammonia Unit. The leak was identified during a routine morning inspection by the shift operator. The area was evacuated following SSHE protocol and a temporary clamp was applied. No injuries were reported. The preliminary assessment suggests flange gasket degradation due to thermal cycling.
Preliminary Investigation — Ammonia Vessel V-101 Overpressure
On 10 April 2026 at 03:00, the pressure relief valve on ammonia storage vessel V-101 lifted following a process upset. The release was directed to the flare as designed. No personnel were present in the exclusion zone at the time. The process control team has identified a potential instrumentation failure as a contributing factor. A full root cause investigation has been initiated.
